Roles & Responsibilities

• Coordinate patient care across the Surgery Outpatient Clinic, ensuring smooth transitions and timely follow-up
• Collaborate closely with nursing staff and providers to create and implement case management plans
• Manage outpatient case loads with attention to detail, prioritizing patient safety and outcomes
• Communicate effectively with patients, families, and the care team to clarify care plans and next steps
• Document care coordination activities and maintain accurate patient records in a timely manner
• Support clinic workflows to help reduce barriers to care and improve patient experience

Qualifications

• Active Registered Nurse (RN) credential (as role title indicates)
• Minimum three (3) years of case management experience (required)
• Experience in outpatient clinic or surgery clinic settings preferred
• Strong collaboration and communication skills with providers and nursing teams
• Ability to work the posted weekday daytime schedule and attend scheduled orientations/compliance deadlines

Knowledge & Skills

• Case management and care coordination best practices
• Patient-centered care and outpatient clinic workflows
• Clinical documentation and care plan management
• Ability to communicate and collaborate with multidisciplinary teams
• Familiarity with surgical outpatient processes and patient follow-up
