Physician Assistant/Nurse Practitioners
Physician Assistants and Nurse Practitioners Who Lead with Purpose - We're Looking for You
At Integrity Medical Post Acute Care Specialists (IMPAC), we are redefining what post-acute care should feel like - for patients, families, and the providers who serve them.
We are a physician-led organization built on a simple belief: the best care happens when clinical excellence meets integrity, communication, and compassion.
We serve patients across Skilled Nursing Facilities (SNFs), rehabilitation centers, and Long-Term Acute Care facilities (LTACs) - at one of the most critical points in healthcare: the transition after hospitalization, where outcomes, trust, and quality of life are truly shaped.
The Opportunity
We are seeking Physician Assistants (PA-C) and Nurse Practitioners (NP) to join our growing team across post-acute care settings. In this role, you will:
- Manage medically complex, high-acuity patients across SNF and LTAC settings
- Conduct admissions, rounding, and ongoing follow-up visits
- Partner with physicians, PAs and NPs to deliver team-based, coordinated care
- Ensure CMS compliance while maintaining a patient-first approach
- Build meaningful relationships with patients, families, and facility partners
- Contribute to quality improvement and value-based care initiatives
Why PAs and NPs Choose IMPAC
- Competitive compensation + productivity incentives
- Malpractice coverage provided
- Full benefits (health, dental, vision) for full-time providers
- W2 and 1099 options available
- Strong administrative and clinical support
- True interdisciplinary collaboration
- Leadership and Medical Director opportunities
- Real influence over care models and workflows
What Sets Us Apart: Our Culture We are not just building a company - we are building a standard.
We believe:
- Integrity is not optional
- Communication is clinical care
- Families are part of the care team
- The "extra step" is the expectation
We practice medicine the way it was meant to be: thoughtful, relationship-driven, outcome-focused, and ethical.
You're a Fit Here If You Believe
- Patients deserve dignity - especially in their most vulnerable moments
- Families deserve honest, consistent communication
- Healthcare should feel human, not transactional
You'll join a team that treats patients like family, values integrity over volume, communicates when others don't, and shows up - consistently.
Qualifications
- Licensed Physician Assistant (PA-C) or Nurse Practitioner (NP) - Massachusetts license or eligible
- Experience in SNF, LTAC, or hospital medicine preferred
- Strong clinical and documentation skills
- Commitment to high-quality, patient-centered care
