Oxford, New York - Post-Acute Care NP/PA Job Opportunity

Role Overview

• Location: Oxford, NY

• Schedule: Part-time, 2 days per week (Tuesdays & Thursdays)

• Setting: Post-acute inpatient / skilled nursing facility

What You'll Do

• Deliver comprehensive primary care to post-acute patients

• Manage both acute and chronic medical conditions

• Develop and implement individualized care plans

• Collaborate closely with the interdisciplinary team (IDT)

• Partner with and work alongside a physician

What We're Looking For

• Experienced clinicians preferred

• Certified FNP, CNP, AGNP, APRN, or Physician Assistant

• Active and unrestricted New York NP or PA license

• Active DEA license

Compensation

• Productivity-based compensation model (fee-for-service)

• Estimated annual compensation: $49,096 - $58,917
