FOX Rehabilitation is seeking a motivated and compassionate Physical Therapist to join our clinician-owned and operated private practice serving older adults throughout the Greater Jersey City, NJ area. As a Physical Therapist with FOX, you will provide evidence-based, geriatric-focused rehabilitation services designed to improve functional mobility, safety, independence, and overall quality of life for your patients in their homes, assisted living communities, and other senior care settings.
In this role, you will evaluate older adult patients, develop individualized plans of care, and deliver skilled physical therapy interventions including therapeutic exercise, balance and gait training, neuromuscular re-education, transfer and stair training, fall-prevention strategies, and home safety recommendations. You will apply your clinical judgment to perform comprehensive examinations, interpret findings, set measurable functional goals, and regularly reassess and progress treatment based on patient response and best available evidence. Your practice will emphasize functional, real-world activities that matter most to your patients, from safe ambulation and transfers to participation in daily routines and meaningful life roles.
You will work collaboratively within an interdisciplinary team that may include occupational therapists, speech-language pathologists, physicians, nurses, social workers, and family caregivers. Effective communication and thorough, timely documentation using electronic medical records are essential, as you will coordinate care, report progress, and support continuity of services. You will educate patients and caregivers on home exercise programs, proper body mechanics, safety strategies, and condition management to promote long-term success beyond formal therapy.
