The Speech Language Pathologist is responsible is to provide direct and indirect speech language therapy services primarily on a visiting and intermittent basis in the patient's home in accordance with the policies and procedures of Catholic Home Health Services of Broward. Knowledge & Experience Requirements • Clear and Active Speech Language Pathology license to practice in the state of Florida. • CPR certification. • Bachelor's or Master's Degree in Speech-Language Pathology. • 2 years clinical experience preferred/ • CCC required or currently completing CFY to obtain CCC. • 1-2 years of experience with a Medicare Certified Home Health agency preferred. • Knowledge of AHCA regulations, Medicare COP's guidelines required. • Must have knowledge of computer office/clinical software. • Must be able to read, write and understand the English language. Essential Functions • Evaluate, direct and provide speech/language pathology service to patients in the home or facility. • Participate in the development and periodic review of the Plan of Care (POC). • Performs assessments and treats disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders and conditions. • Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening. • Observe, record and report changes in the patient's condition and response to treatment to supervisor and/or the physician. • Provide instruction and training to patients in use of alternative communication systems when appropriate. • Provide counsel and instruction to patients, families and healthcare staff. • Participate in care coordination activities and discharge planning. • Apply concepts of agency's infection control plan and universal precaution in coordination/performing client cares activities to protect both patient and staff (OSHA).
