Job Summary

Trinity Home Health is a boutique home health agency dedicated to providing personalized, high-quality care in patients' homes. We are seeking an Occupational Therapist (OT) to join our team on a per diem, pay-per-visit basis.

The Home Health OT will evaluate patients, develop individualized treatment plans, and deliver skilled occupational therapy services aimed at improving independence, safety, and overall quality of life. This role is ideal for a clinician who enjoys autonomy, flexibility, and collaborative patient care.

Key Responsibilities

• Perform occupational therapy evaluations and ongoing reassessments in patients' homes
• Develop and implement individualized plans of care in collaboration with physicians and the care team
• Provide skilled interventions to improve activities of daily living (ADLs), functional mobility, and home safety
• Recommend adaptive equipment and environmental modifications as appropriate
• Educate patients and caregivers on therapeutic techniques, safety strategies, and home programs
• Accurately document visits and patient progress in the agency's EMR system
• Complete OASIS assessments as needed
• Communicate patient status and progress with physicians and interdisciplinary team members
• Ensure compliance with home health regulations and agency policies

Qualifications

• Current California Occupational Therapist (OT) license
• Minimum of 1 year of home health experience required
• OASIS and EMR experience preferred
• CPR certification
• Strong clinical reasoning, communication, and documentation skills
• Ability to work independently and manage a field-based caseload
• Reliable transportation and a valid driver's license
