Cherrelyn Healthcare Center, part of the Stellar Living family, is seeking a compassionate and licensed Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide high‑quality, evidence‑based rehabilitative care to a diverse population of residents in a skilled nursing and post‑acute setting, helping them regain function, reduce pain, and improve overall quality of life. You will perform comprehensive evaluations, develop individualized treatment plans, deliver therapeutic interventions, and regularly reassess progress to ensure that each resident’s goals are met safely and effectively. As a Licensed Physical Therapist at Cherrelyn Healthcare Center, you will collaborate closely with an interdisciplinary team—including nursing, occupational therapy, speech therapy, social services, and medical providers—to coordinate care and support optimal clinical outcomes. Your day‑to‑day responsibilities will include conducting mobility and strength assessments, designing and progressing exercise and gait training programs, providing balance and fall‑prevention interventions, educating residents and families on home exercise and safe mobility strategies, and completing all documentation in accordance with regulatory, payer, and facility standards. You will contribute to discharge planning, durable medical equipment recommendations, and care conferences, ensuring that residents have the support and resources needed to transition safely across care settings.
