Definitions

Functions
Area of Responsibilities
*Initiate the birth certificate process by completing vital record reports and forms. Ensure compliant completion of paternity acknowledgement forms, if applicable. Ensure that all required documentation for each newborn is transmitted via electronic submission in accordance with state law. Ensure each newborn name is updated in the electronic medical record (Epic) following hospital policy once newborn name has been confirmed and reported.
*Process pronouncement of death forms each workday and fax the form to the appropriate funeral home within 48hrs or 2 business days.
*Ensure Release of Body forms are uploaded to the proper encounter in the electronic medical record affiliated with an expired discharge disposition, when applicable
*Report notifiable birth defects to the Georgia Birth Defect Registry monthly. Generate Epic report to identify applicable birth encounters that must be reported within 30 days of diagnosis. The Birth Defect Reporting Form provided by the GA DPH must be used based on Tanner's low volumes.
*Report Spontaneous Abortion data each workday to the Georgia and Alabama Fetal Death registries according to state rules and regulations. Generate Epic report to identify patients with products of conception diagnosis and report spontaneous abortions within 48 hours 2 business days of the occurrence. The Fetal Death registration form template from DPH must be used to complete the Spontaneous Abortion reporting.
*Report monthly listing of deaths, THS patient encounters with an expired discharged disposition, to Georgia and Alabama's Organ & Tissue organizations. Generate Epic reports to compile complete list. Communicate with both Organ & Tissue organizations monthly to confirm report was received timely via secure email.
*Serve as a Medical Certifier User authorized by Georgia to certify death certificates initiated by funeral facilities in the GA Vital Event Registration System (GAVERS), if needed. This role requires registration and confidentiality agreements issued by the Georgia State Government.
*Ensure electronic submission of required data or protected health information to various state portals compliantly and timely.
*Completes vital record reports and forms at an overall accuracy rate of 99%. Ensures that information is transmitted in accordance with State law.
*Assist visitors, patients, physicians, and other hospital staff whether through direct contact or by telephone communication.
*Completes daily volumes to ensure the HIM Operations team and HIM Department meets assigned goals.
*Ensures compliance with external regulatory agency requirements such has those from the Joint Commission, CMS, RAC, OIG, etc.
*Performs all other duties and tasks assigned.
Compliance Statement
