What You'll Do (Full-Time Role):
Provide full-time skilled physical therapy services in patients' homes
Evaluate mobility, strength, balance, and functional limitations
Develop and implement individualized treatment plans
Educate patients and caregivers on home exercise programs and safe mobility techniques
Monitor progress, document outcomes, and adjust treatment as needed
Collaborate with an interdisciplinary care team to ensure coordinated, high-quality care
What You'll Need:
Active Florida Physical Therapist (PT) license
Degree from an accredited Physical Therapy program
Valid driver's license & reliable transportation
Current CPR certification
Strong communication, documentation, and time-management skills
Home health experience (preferred)
