Key Responsibilities

• Clinical Excellence: Conduct comprehensive initial evaluations and develop evidence-based, individualized treatment plans.
• Therapeutic Intervention: Deliver high-quality manual therapy, therapeutic exercises, and mobility training tailored to the home environment.
• Empowerment: Educate patients and their families on safety, home exercise programs (HEPs), and long-term recovery strategies.
• Documentation: Maintain timely and accurate clinical records via our streamlined documentation process to track progress and ensure compliance.

What You Bring to the Team

• Licensure: An active, unrestricted license to practice Physical Therapy in the State of Maryland.
• Experience: Previous experience in Home Health is preferred, but we value strong clinical skills and a passion for geriatric and rehabilitative care.
• Communication: Exceptional interpersonal skills with the ability to build rapport with patients, families, and the interdisciplinary team.
• Reliability: A valid driver’s license, reliable transportation, and a self-starting "can-do" attitude.
