Key Responsibilities
- Provide evidence-based primary care to adult patients and geriatric patients.
- Conduct preventive exams, chronic disease visits, acute care, and follow-ups.
- Review labs, imaging, consult notes, and medication refills in a timely manner.
- Support care coordination and ensure continuity of care with specialists and hospitals.
- Collaborate with the CEO and clinical team on quality improvement, patient safety, and HRSA compliance measures.
- Participate in chart reviews, peer review, and clinical policy development as needed.
- Maintain accurate and timely documentation in the EHR.
- Uphold Apexs mission of accessible and compassionate care.
- MD or DO, Board Certified/Board Eligible in Internal Medicine or Family Medicine
- Active California medical license
- DEA license required
- Experience in community health, FQHC, or underserved populations strongly preferred
- Bilingual (Armenian or Spanish) is highly desired but not required
