In this role, you will be based in our clinic located in Lakeland, Florida, reporting directly to an associate medical director. Utilize Your Skills to Make a Difference
Your Responsibilities
  • Deliver comprehensive care to seniors in accordance with clinical standards.
  • Engage daily with your care team to ensure coordinated, high-quality service.
  • Manage referrals, coordinate with hospitals/SNF, and oversee DME and home health services.
  • Participate in daily team huddles.
  • Document patient care with assistance from quality-focused coders.
  • Join the on-call rotation and support growth initiatives at your center.
  • Ensure adherence to all licensing and accreditation standards.
What We Are Seeking

Required Qualifications:

  • Master’s degree as a nurse practitioner or completion of a physician assistant program with board certification.
  • Active, unrestricted NP or PA license in the state where you will practice.
  • Minimum of two years of experience in value-based care, geriatric, or primary care, specifically managing adults aged 65 and older with chronic conditions.
  • Proficiency in computer systems (EMR, email, etc.).
  • This position involves patient interaction and is part of our Tuberculosis (TB) screening program. If selected, you will need to undergo TB screening.

Preferred Qualifications:

  • Experience in Medicare Advantage environments.
  • Possession of an active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.
  • Familiarity with HEDIS quality indicators and Medicare regulations.
Additional Information

Scheduled Weekly Hours: 40

Compensation Range: The salary range provided reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.

$123,800 - $170,300 per year

This position is eligible for a bonus incentive plan based on individual and/or company performance.
