Overview of the Role and QualificationsAs a Case Manager, you will be instrumental in guiding patients and their families throughout their healthcare journey. Your responsibilities will encompass coordinating services, connecting resources, and crafting care plans tailored to the individual needs of each patient. By collaborating with physicians, nurses, and departmental leaders, you will help facilitate safe transitions, ensure effective communication, and maintain high-quality care across the entire continuum.
Your key responsibilities will include:
- Assisting patients and families during program orientation by explaining rehabilitation philosophies, Medicare and insurance benefits, discharge criteria, and patient rights.
- Coordinating educational initiatives for patients, families, and caregivers to foster involvement in treatment planning, goal-setting discussions, and family meetings.
- Creating and executing personalized treatment plans that align with the patient’s strengths, needs, and recovery objectives.
- Conducting psychosocial assessments and discharge planning evaluations, thoroughly documenting findings and communicating needs to the care team.
- Working closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure cohesive, patient-focused care.

- Registered Nurse (RN)
- Bachelor’s Degree, Master’s Degree, or Registered Nurse Diploma
- No travel is required.
- A minimum of 3 years of relevant experience is necessary.
