        • Current Inpatient Case Management experience required
        • Acute Care Hospital experience preferred
        • Experience with discharge planning and care coordination
        • Experience using Epic EMR
        • Experience with post-acute referral platforms such as NaviHealth or AllScriptsRequired Certifications
          • Active RN License
          • BLS Certification
          • Current CPR Card (Required for Bedside Case Managers)Required Skills

              • Coordinate care for assigned adult medicine and surgical patients throughout hospitalization
              • Collaborate with physicians, nurses, social workers, therapists, and interdisciplinary teams
              • Develop and implement safe discharge and transition-of-care plans
              • Coordinate placement to skilled nursing facilities, acute rehabilitation facilities, long-term care settings, or community resources
              • Communicate with insurance payors to obtain treatment approvals and necessary resources
              • Monitor clinical, operational, and fiscal indicators related to patient care
              • Facilitate patient and family participation in discharge planning
              • Maintain accurate patient records and documentation in Epic and departmental systems
              • Ensure compliance with hospital policies, regulatory standards, and quality initiatives
