Our organization is actively seeking a Board-Certified or Board-Eligible Family Medicine or Internal Medicine Physician to join our team at the Center of Hope in Newark, DE. This role is essential for enhancing primary care access and expanding services within the Delaware region, particularly for the medically underserved populations, with a special emphasis on the Latino community.

Practice Overview

Our full-service Family Medicine Center is dedicated to providing comprehensive primary care. We prioritize cultural competency in our approach, supported by a bilingual team that is committed to delivering inclusive, patient-centered care.

Key Responsibilities
  • Build and develop a strong patient panel.
  • Engage in marketing initiatives to foster practice growth.
  • Receive organizational support for community engagement and practice expansion.
  • Manage patient volume effectively.
  • Utilize an integrated Epic EMR system.
  • Leverage a robust referral network within our organization.
Qualifications
  • MD or DO degree from an accredited institution.
  • Preferred Board Certification or Eligibility in Family Medicine.
  • Eligibility for licensure in Delaware.
  • Dedication to providing high-quality, patient-centered care.
Role Responsibilities
  • Deliver comprehensive primary care to patients of all ages.
  • Diagnose and manage both acute and chronic health conditions.
  • Focus on preventive care and population health management.
  • Collaborate with interdisciplinary teams to enhance patient outcomes.
  • Ensure timely and accurate documentation of patient interactions.
  • Participate in initiatives aimed at quality and performance improvement.
Schedule & On-Call Duties
  • Full-time schedule with 36 patient-facing hours.
  • Flexible scheduling options available.
  • On-call responsibilities on a 1:2 call schedule, phone-only.
