Job Opportunity: Physical Therapist

Our organization is in search of a committed and empathetic Physical Therapist to become a vital part of our Healthcare & Medical Services team. In this position, you will deliver exceptional, evidence-based rehabilitation services to patients across various care settings, including skilled nursing facilities, outpatient clinics, and home health environments, based on the needs of the organization.

As a Physical Therapist, your responsibilities will include:

  • Conducting thorough evaluations and assessments of patients.
  • Creating personalized care plans tailored to individual patient needs.
  • Implementing therapeutic interventions that adhere to best clinical practices and regulatory guidelines.
  • Educating patients, families, and caregivers on home exercise routines, safety measures, and wellness strategies to promote long-term health.
  • Regularly reviewing patient progress, adjusting treatment plans as necessary, and accurately documenting all services in our electronic medical record system.

You will work collaboratively with a multidisciplinary team, including physicians, nurses, occupational and speech therapists, social workers, and other healthcare professionals. Your role will involve:

  • Coordinating patient care and participating in care plan meetings.
  • Contributing to discharge planning and advocating for patient needs.
  • Maintaining clear and professional communication with all team members.
