The Medical Social Worker, under the supervision of the Clinical Manager, plays a vital role in the interdisciplinary care provided to patients receiving home health services. This position involves assessing and crafting individualized care plans that address the emotional and social needs of patients. Additionally, the Medical Social Worker will oversee and guide the Social Worker Assistant involved in the patient's care plan. Adherence to our organization's policies, healthcare standards, and relevant laws and regulations is essential.
Key ResponsibilitiesTB Screening: This position involves direct patient interaction and is part of our Tuberculosis (TB) screening program. Candidates selected for this role will be required to undergo TB screening.
Driving Requirements: This role is included in our driver safety program, necessitating a valid state driver's license and personal vehicle liability insurance. Individuals must maintain insurance that meets or exceeds state minimum requirements, or $25,000 for bodily injury per person, $25,000 for bodily injury per event, and $10,000 for property damage, or higher.
Work Schedule and CompensationThe estimated pay range for this full-time position (40 hours per week) is between $53,700 and $72,600 annually. This range reflects a good faith estimate at the time of posting and may vary based on geographic location, individual skills, experience, education, and certifications.
