Job Description

Job Title: Transition Coordinator - Wage/Hour Status: Exempt

Reports to: Executive Director of Special Education - Pay Grade: Pay Grade 3 (197 days) $53,131-$83,233

Dept./School: Special Education - Date Revised: January 2021

Primary Purpose:

This employee will work with students, staff, parents, and community to plan, implement and evaluate a comprehensive developmental guidance and transition program on all campuses. Special education students will be counseled and vocational assessed to fully develop academic, career, personal, and social abilities as they transition from public school to post-secondary settings. This employee will facilitate coordination with agencies that can provide post-secondary assistance to achieve student's goals.

Qualifications:

Education/Certification:

Bachelor's degree from accredited university

Valid Texas teaching certificate in Special Education

Special Knowledge/Skills:

Knowledge of vocational skills assessment, student appraisal and career development

Ability to instruct students and manage their behavior

Excellent organizational, communication, and interpersonal skills

Experience:

Three years teaching Special education

Teaching vocational experience

Major Responsibilities and Duties:

Planning:

  1. Guide individuals and groups of special needs students to develop education plans and career awareness.

Consultation:

2. Consult with parents, teachers, administrators, agencies and other relevant people to enhance work with students.

  1. Identify and work with school and community personnel to bring together resources for students.
  2. Maintain a communication system that effectively collects and disseminates information to other professionals as appropriate.
  3. Develop and maintain positive working relationships with other school professionals and representatives of community resources.
  4. Use an effective referral process to assist special needs students to use available programs and services.

Assessment:

  1. Facilitate vocational testing for special needs students as part of transition planning.
  2. Interpret tests and other appraisal results appropriately in order to communicate to school personnel, students, and their parents.
  3. Maintain confidentiality of student assessment.

Program Management:

  1. Plan transition program to ensure that it meets identified needs, priorities, and State Performance Plan objectives.
  2. Implement a comprehensive and balanced program.
  3. Create, compile, maintain, and file all required physical and computerized reports, records and other documents.
  4. Attend and participate in ARDs as requested/required.

Administration:

  1. Comply with policies established by federal and state law, State Board of Education rule, and board policy transition area.
  2. Comply with all Student Support Services routines and regulations.
  3. Maintain a positive and effective relationship with campus administrators.
  4. Communicate effectively with colleagues, students, and parents.

Professional:

  1. Participate in professional development to improve skills related to job assignment.
  2. Attend and participate in Student Support Services meetings, activities and training.

Supervisory Responsibilities:

Supervise assigned teacher aide(s), if applicable.

Working Conditions:

Mental Demands/Physical Demands/Environmental Factors:

Maintain emotional control under stress. Frequent standing, stooping, bending, pulling and pushing. Move small stacks of textbooks, media equipment, desks, and other classroom equipment.
