Position Overview: The Nutrition Services Coordinator assists in new client intake processes by conducting malnutrition risk screenings and scheduling nutrition counseling appointments for Registered Dietitian Nutritionists. They also assist in ordering and stocking of oral nutrition supplements and client outreach projects as needed. The Nutrition Services Coordinator will be an integral part of the Nutrition team helping to provide outreach and education to clients and external partners, and assist in departmental tasks as needed.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Completes risk screening of new clients by phone and mail; assists with prioritization of clients by communicating risk and MNT interest with Registered Dietitian Nutritionists.
  • Coordinates scheduling of nutrition assessments.
  • During client screening, reviews client meal and supplement orders.
  • Conducts outreach to clients’ health care providers as needed to gather additional information about clients.
  • Maintains accurate case notes and documentation of client information and services.
  • Prepares reports regarding client services including data entry into CAREWare, Client Track, and collection of client outcomes data.
  • Receives, sorts and packages nutritional supplements for delivery.
  • Assists the Nutrition Services Manager and Registered Dietitian Nutritionists on projects, surveys, studies, outreach, and other tasks.
  • Performs clerical and administrative support for the Nutrition Department.
  • Assists in the development and delivery of nutritional education materials, including, but not limited to handouts, videos, presentations, and cooking demonstrations.

EDUCATION & EXPERIENCE

  • High school diploma or equivalent required.
  • Dietetic Technician, registered (DTR), preferred.
  • At least 2 years relevant experience, ideally in a community setting and working with diverse populations.
  • Prior experience working in or volunteering at a nonprofit organization.

Or an equivalent combination of education, training, and/or experience from which comparable knowledge, skill, and abilities have been attained.
