As a Registered Dietitian within a PACE (Program of All-Inclusive Care for the Elderly) organization, you will assess participants' nutritional status, develop individualized nutrition interventions, and collaborate with the interdisciplinary team to deliver comprehensive, participant-centered care. This role plays a critical part in supporting frail and elderly participants through nutrition counseling, care planning, and ongoing monitoring to promote health, independence, and quality of life.

Hours per week: 40
Conduct comprehensive nutritional assessments at enrollment and periodic reassessments per PACE regulations
Develop and update individualized nutrition care plans in collaboration with the interdisciplinary team
Provide nutrition counseling and education to participants, caregivers, and families
Monitor clinical indicators including weight, hydration, intake, swallowing/chewing issues, and drug-nutrient interactions
Recommend and coordinate nutrition-related interventions such as meal services and supplements
Participate in interdisciplinary team meetings, care conferences, and quality improvement initiatives
Maintain accurate documentation and ensure compliance with safety and infection control protocols
Strong clinical assessment and nutrition care planning skills
Knowledge of geriatric nutrition, chronic disease management, and drug-nutrient interactions
Excellent communication, counseling, and interdisciplinary collaboration skills
Strong documentation, organizational, and patient-centered care approach
