Job Description

Position Summary

Acadian Health is seeking an experienced Community Paramedic to provide hospital-level care in patients' homes as part of our Hospital-at-Home program. This role combines advanced clinical skills, care coordination, patient education, and technology to support patients recovering from acute illnesses and medical conditions outside of a traditional hospital setting. The Community Paramedic serves as the in-home clinical partner to a virtual care team, helping ensure safe, effective, and high-quality patient care.

Who Will Be Successful in This Role?

The ideal candidate is an experienced paramedic who is comfortable working independently in patients' homes, collaborating virtually with healthcare teams, and providing advanced patient care outside of a traditional hospital setting. Community Paramedicine, Critical Care, or Mobile Integrated Healthcare experience is highly valued.

This position requires current Paramedic licensure and experience providing direct patient care in emergency, acute care, or pre-hospital settings. Candidates who are not currently licensed as a Paramedic or who lack field-based patient care experience may not be considered.

Schedule: 2 Days On | 2 Days Off | 3 Days On

Minimum Qualifications (Required)

Please apply only if you meet the following requirements:

  • High School Diploma or GED
  • Current state Paramedic license/certification
  • Current BLS and ACLS certifications
  • Valid driver's license with an acceptable driving record
  • Ability to obtain one of the following within two years of hire:
    • Community Paramedicine (CP-C) certification, or
    • NEMSA Essentials Course completion, or
    • Community Paramedic licensure as required by the state
  • Strong communication and patient education skills
  • Ability to work independently while collaborating with a remote healthcare team
  • Comfort with electronic documentation and basic computer applications

Preferred Qualifications

  • Associate's or Bachelor's degree
  • Previous Critical Care or Mobile Integrated Healthcare experience
  • Community Paramedicine experience
  • Instructor, educator, or preceptor experience
  • PHTLS and PALS certifications

Key Responsibilities

In-Home Patient Care

  • Deliver hospital-level care in patient homes for conditions such as infections, heart failure, pneumonia, and post-surgical recovery
  • Perform physical assessments, treatments, and clinical interventions according to physician orders and established protocols
  • Monitor patient status and identify changes that require escalation or intervention

Care Coordination

  • Collaborate with physicians, nurses, advanced practice providers, case managers, and other healthcare professionals
  • Serve as the on-site extension of the virtual care team
  • Communicate patient updates and clinical findings in real time to support care decisions

Patient & Caregiver Education

  • Educate patients and families on medications, treatment plans, disease management, and recovery expectations
  • Reinforce discharge instructions and promote adherence to care plans
  • Help patients recognize when additional medical attention is needed

Documentation & Compliance

  • Document assessments, treatments, and patient interactions accurately in the electronic health record (EHR)
  • Maintain compliance with all program protocols, quality standards, and regulatory requirements
  • Ensure timely and complete documentation of patient care activities

Emergency Response & Clinical Support

  • Respond to urgent clinical situations in the home environment
  • Perform appropriate interventions within scope of practice and program protocols
  • Support continuity of care and help prevent unnecessary emergency department visits or hospital readmissions

Program & Team Support

  • Participate in quality improvement initiatives and program development activities
  • Assist with onboarding, mentoring, or training of team members as needed
  • Represent Acadian Health professionally within the community and healthcare system
