The Sub-Acute and Long-Term Care sector is rapidly growing, offering excellent opportunities and a flexible work-life balance (no on-call duties or weekend shifts)!
We are looking for a full-time provider (40 hours per week) for our facility located in Santa Fe, NM. This role is open to either a nurse practitioner or a physician assistant, and we encourage new graduates to apply.
The salary for this position ranges from $115,000 to $125,000 per year, along with an incentive compensation bonus program that has no cap.

Key Responsibilities

The Advanced Practice Provider will report to the Director of Clinical Practice and/or the Area Clinical Practice Manager. This role involves working closely with the Center Leadership Team to meet specific center goals and priorities. Additionally, the Advanced Practice Provider will collaborate with the Medical Director and/or Attending Physician to deliver patient-specific medical care.

  • Provide primary and urgent care services to residents in the nursing center.
  • Work in partnership with other medical providers to enhance the quality of care.
  • Collaborate with Center Leadership to fulfill our mission of improving lives through high-quality healthcare and compassion.
Qualifications

The ideal candidate must have graduated from an accredited School of Nursing and hold a valid registered nurse license. For nurse practitioners, certification is required from an accredited Nurse Practitioner Program with a Master’s Degree in Nursing, focusing on Older Adult and/or Family Nurse Practitioner (FNP). Other specialties may be considered based on experience and organizational needs. For physician assistants, graduation from an ARC-PA accredited program and successful completion of the PA National Certifying Exam (PANCE) is necessary, along with state licensure and NCCPA certification.

A minimum of two years of experience working with adult or geriatric populations is preferred.

In states where practice regulations permit, the Advanced Practice Provider should possess an unrestricted professional license and DEA for each state of practice.
