Key Responsibilities
  • Conduct comprehensive evaluations of patients, including the review of medical histories and performance of physical assessments to determine appropriate treatment plans.
  • Clearly communicate evaluation results and proposed treatment strategies to patients and their families, fostering a collaborative goal-setting process.
  • Design and implement individualized treatment protocols based on assessment findings and patient progress, ensuring that care is tailored to each patient's unique needs.
  • Engage in regular consultations with other healthcare professionals to coordinate care, document changes in patients' conditions, and modify treatment plans as necessary.
  • Manage discharge planning and follow-up care, ensuring thorough documentation of the patient's journey, including initial evaluations, progress notes, and discharge summaries.
  • Maintain high standards of productivity by effectively scheduling patient appointments and participating in other hospital or community-related activities.
  • Contribute to departmental goals by adhering to regulations and protocols, while actively engaging in initiatives to improve patient access to care.

Qualifications

Compensation and Benefits

This full-time position offers a competitive salary ranging from $100,000 to $120,000 annually, commensurate with experience.
