Key Responsibilities
  • Examine, evaluate, and develop individualized treatment plans for patients suffering from functional limitations or disabilities, ensuring alignment with state practice Acts.
  • Conduct thorough evaluations that include a review of medical history, assessments, and document a clear plan of care that highlights goals, baseline, and historical data.
  • Utilize evidence-based practices to implement treatment protocols effectively, adapting strategies in response to specific patient presentations and changes in condition.
  • Collaborate with healthcare team members as needed, making timely adjustments to treatment plans and consulting on additional medical needs where appropriate.
  • Document patient care activities accurately and in compliance with evolving healthcare regulations, including daily encounter notes and detailed discharge planning.
  • Engage in continuous training to uphold state/local/federal standards, enhancing both clinical and corporate service competencies.
  • Participate in interdepartmental meetings and contribute actively to initiatives aimed at removing barriers to patient access and improving service delivery.
Patient Population / Client Focus

This role specifically focuses on serving patients within the home care sector, providing therapy for individuals with orthopedic needs. Qualifications
  • Doctorate in Physical Therapy (DPT) from an accredited institution.
  • Active State License as a Physical Therapist in New Jersey.

  • Current CPR certification for professional healthcare providers.
Preferred Experience
