Work EnvironmentThis position is entirely onsite, within a home care setting. You will work closely with an interdisciplinary team of healthcare professionals, including physicians, nurses, and occupational therapists. Key Responsibilities- Conduct comprehensive evaluations of residents to accurately assess their physical capabilities and specific therapy needs.
- Design and implement customized treatment plans based on assessments, aimed at enhancing residents' strength, mobility, and functional independence.
- Administer therapeutic exercises, manual therapy, and utilize various modalities to effectively address physical impairments.
- Monitor and document residents' progress meticulously, making adjustments to treatment plans as needed to ensure optimal outcomes.
- Engage in collaborative efforts with the healthcare team to ensure a cohesive approach to care, keeping all members informed on patient progress.
- Educate residents and their families on effective home exercise programs and strategies to maintain and enhance physical health.
- Maintain compliance with all state and federal regulations as well as professional standards in therapeutic practice.
The primary focus of this role is on the geriatric population, often residing in our skilled nursing facility. You will work closely with individuals recovering from surgeries, strokes, or other debilitating conditions, providing compassionate care that not only addresses their physical rehabilitation needs but also promotes overall quality of life. By tailoring your approach to each resident's unique circumstances, you contribute significantly to their recovery journey.
Qualifications- Current licensure as a Physical Therapist in the state of California is mandatory.
- A Bachelor's degree in Physical Therapy from an accredited institution is required; a Master's or Doctorate is preferred.
- Strong communication and interpersonal skills, facilitating effective interactions with residents, families, and fellow team members.
- Ability to work collaboratively in a dynamic team setting while demonstrating strong organizational skills.
- A commitment to ongoing professional development and a desire to stay current with the latest practices in physical therapy.
- Experience working in a skilled nursing facility focused on geriatric care is highly desirable.
- Proficiency in collaborating with multidisciplinary teams to optimize patient care.
