Work EnvironmentLocated in beautiful Holden, MA, our rehabilitation center fosters a collaborative and supportive culture where transparency and communication are paramount. As a Physical Therapist in our clinic, you will benefit from a structured work schedule of Monday to Friday, from 9 AM to 5 PM, allowing for a balanced professional and personal life. Key Responsibilities- Conduct thorough evaluations of patients' physical abilities to identify functional limitations and create personalized treatment plans that cater to their unique rehabilitation goals.
- Provide skilled and compassionate physical therapy interventions to geriatric patients, including therapeutic exercises, manual therapy techniques, and the application of other modalities to enhance recovery.
- Regularly monitor and document each patient's progress, utilizing EMR systems, and adjust treatment plans as necessary to achieve optimal patient outcomes.
- Collaborate with an interdisciplinary team of healthcare professionals, including physicians, occupational therapists, and speech therapists, to ensure a coordinated and comprehensive approach to patient care.
- Educate patients and their families about therapeutic techniques, home exercise programs, and safety strategies to promote ongoing health and mobility.
- Implement innovative fall prevention programs tailored for the assisted living environment, effectively reducing the risk of accidents and ensuring patient safety.
- Stay abreast of the latest advancements in physical therapy through continuous education, workshops, and discussions, to improve treatment methodologies.
- Uphold ethical and professional standards in your practice, maintaining confidentiality and ensuring the highest levels of care.

Qualifications- A graduate degree from an accredited institution in Physical Therapy, holding either a Doctorate, Master's, or Bachelor's degree.
- An active Physical Therapist licensure in the state of Massachusetts.
- Demonstrated strong interpersonal and communication skills, facilitating effective interactions with patients, their families, and other healthcare professionals.
- Proficiency in electronic medical records systems to ensure efficient documentation of patient progress and treatment plans.
- A commitment to evidence-based practices, underscoring your ability to apply the latest therapeutic techniques tailored to individual patient needs.
- While a minimum of one year of experience in geriatric therapy is preferred, we also welcome applications from new graduates passionate about aging-related care.
