Key Responsibilities
  • Conduct comprehensive assessments of residents to evaluate their physical condition, functional capabilities, and specific rehabilitation needs.
  • Develop and implement individualized treatment plans that focus on enhancing mobility, reducing discomfort, and fostering independence in activities of daily living.
  • Utilize various therapeutic modalities—including but not limited to therapeutic exercises, manual techniques, and balance training—to restore and improve physical function.
  • Educate residents, caregivers, and families about mobility techniques, safety measures, and effective home exercise programs that facilitate continuity of care.
  • Monitor and document patient progress meticulously, adjusting treatment strategies as necessary based on their recovery trajectory.
  • Work collaboratively with a multidisciplinary healthcare team, including nurses, occupational therapists, and case managers, to ensure comprehensive, patient-centered care.
  • Ensure compliance with all healthcare regulations, including HIPAA, while maintaining accurate clinical records and documentation.
Patient Population / Client Focus

The Physical Therapist will primarily work with assisted living residents who often face complex rehabilitation challenges due to age-related conditions, post-surgical recovery, or chronic illnesses. Our patient population includes older adults who require specialized therapies to regain strength and functionality, as well as younger patients undergoing rehabilitation following injuries. Your contributions will directly influence their ability to engage in meaningful activities and improve their overall well-being.

Qualifications
  • A valid and active Physical Therapist license in the state of Texas.
  • A Bachelor's Degree in Physical Therapy from an accredited institution; candidates with a Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) are preferred.
  • Minimum of two years of clinical experience in physical therapy practice.
  • Proficiency in conducting manual therapy, therapeutic exercises, and balance training.
  • Exceptional communication and interpersonal skills with the ability to build rapport with residents, families, and multidisciplinary teams.
  • Familiarity with electronic health records (EHR) systems is considered an asset.
Preferred Experience
