Position Overview

Work Environment

Our outpatient office is designed to create an inviting and efficient treatment space. Operating Monday through Friday, from 8:00 am to 5:00 pm, we offer a structured schedule that supports work-life balance. Key Responsibilities

As a Physical Therapist at Right Form Rehabilitation & Wellness Center, your day-to-day responsibilities will include:

• Conducting thorough examinations and evaluations of patients to develop individualized treatment plans aimed at addressing functional limitations and disabilities.
• Staying well-informed about state practice acts and ensuring compliance with regulations governing physical therapy practice.
• Performing specific assessments, documenting findings, and making necessary recommendations for patient care based on the evaluations conducted.
• Implementing treatment protocols that are carefully tailored to each patient's unique needs and aligning with best practices as informed by the evolving healthcare landscape.
• Collaborating with other members of the healthcare team to recognize changes in patients' conditions, adjusting treatment plans as necessary, and consulting with other professionals when indicated.
• Preparing comprehensive discharge plans and ensuring timely and accurate documentation of patient encounters, progress, and care plans.
• Engaging in continual professional development by attending relevant conferences, in-services, and clinical meetings.
• Removing barriers to patient access and actively participating in initiatives aimed at improving service delivery.
• Providing quality care by following up on treatment efficacy and participating in discussions to enhance service access and outcomes.

Patient Population / Client Focus

Your focus will primarily be on individuals requiring rehabilitation from various conditions, including post-surgical recovery, sports injuries, and chronic pain syndromes. You will engage with a diverse patient population, adapting your therapeutic approach to accommodate varying age groups, cultural backgrounds, and physical abilities. Your expertise will directly contribute to helping each patient achieve their personal health goals.

Qualifications

To qualify for this position, candidates must possess:

• A Doctorate of Physical Therapy (DPT) from an accredited program.
• A valid physical therapy license for the state of New Jersey.
• Strong interpersonal skills, enabling effective communication with patients and the ability to foster rapport quickly.
• A current CPR certification.

Preferred Experience

While a minimum of two years of relevant experience is required, candidates with prior personal training experience or exposure to digital health solutions are encouraged to apply.
