Excelera Physical Therapy is seeking a dedicated and skilled Physical Therapist to join our dynamic rehabilitation team in Port Jervis, NY. Key Responsibilities- Conduct thorough evaluations and assessments of patients' physical capabilities and limitations to develop appropriate treatment plans.
- Administer individualized therapy sessions to an average of seven patients daily, utilizing techniques that encourage mobility, strength, and functionality.
- Educate and instruct patients and their families on therapeutic exercises, treatment plans, and home care advice to promote recovery.
- Maintain accurate and up-to-date patient records, documenting progress and modifications in treatment plans.
- Collaborate with multidisciplinary team members, including physicians, nurses, and occupational therapists, to ensure seamless patient care and communication.
- Participate in regular team meetings and case discussions to review patient progress and integrate feedback into treatment methodologies.
At Excelera Physical Therapy, you will primarily work with a diverse patient population, including older adults recovering from surgeries, individuals with chronic conditions, and patients seeking rehabilitation post-injury. Your role will focus on understanding their unique challenges and tailoring interventions that promote their independence and quality of life.
Qualifications- A valid New York State Physical Therapist License is mandatory.
- A Bachelor's degree in Physical Therapy; a Master's or Doctorate in Physical Therapy is preferred.
- At least two years of experience in a clinical setting is required, demonstrating proficiency in delivering care and therapy.
Experience in a rehabilitation facility or skilled nursing environment is highly advantageous. Familiarity with electronic health record (EHR) systems will also be considered a plus, as it will facilitate the documentation and tracking of patient progress more efficiently.
Compensation and BenefitsWe offer a competitive salary ranging from $100,000 to $120,000 per year, complemented by comprehensive health insurance plans that cater to your medical, dental, and vision needs.
