Key Responsibilities
  • Manage the comprehensive therapeutic care of assigned patients, coordinating with physicians and care team members to ensure that all aspects of care are effectively delivered.
  • Conduct thorough patient assessments and utilize your findings to develop and implement personalized care plans that emphasize goal achievement, patient engagement, and a positive experience.
  • Provide skilled physical therapy interventions in accordance with physician directives, focusing on patient progress and rehabilitation goals.
  • Evaluate and document therapeutic, rehabilitative, and functional statuses while actively participating in the planning and execution of comprehensive care strategies.
  • Deliver therapeutic interventions in patients' homes when required, utilizing a range of techniques and procedures aimed at improving their condition in alignment with the treatment plan.
  • Educate patients, families, and caregivers on effective use of therapies, exercises, and therapeutic devices that can be integrated into daily life outside of therapy sessions.
  • Prioritize your caseload, manage daily staffing requirements, and facilitate seamless transitions and coordination of care for your patients.
Patient Population / Client Focus

You will work primarily with patients recovering from surgeries, injuries, and various orthopedic conditions. Our diverse patient population includes both adults and children, allowing you to utilize a broad range of physical therapy techniques tailored to the specific needs of each age group. You will play a critical role in empowering patients to regain their independence and improve their quality of life through personalized therapy interventions.

Qualifications
  • Current and valid license to practice as a Physical Therapist in the state of Oregon.
  • A Doctor of Physical Therapy (DPT) degree from an accredited institution.
  • Basic Cardiac Life Support (BCLS) certification, to be obtained within 30 days of start date.
  • While no prior experience is required, a strong commitment to patient care and a collaborative attitude are essential.
Preferred Experience
  • Previous experience in an outpatient or rehabilitation setting is beneficial but not mandatory.
  • Familiarity with electronic health record (EHR) systems is advantageous.
Compensation and Benefits
