Work Environment

The role requires you to engage directly with patients in their homes, ensuring a comfortable and recognizable environment for their treatment. Our team operates Monday through Friday from 8:00 AM to 5:00 PM, creating a structured work schedule that promotes work-life balance. As a member of our onsite care facility, you will collaborate closely with other health professionals to provide comprehensive care tailored to each patient's unique needs.

Key Responsibilities
  • Conduct comprehensive examinations and evaluations to develop personalized treatment plans for patients facing functional limitations or disabilities.

  • Adhere to state practice acts and regulations to ensure compliance in the delivery of physical therapy services.

  • Perform in-depth assessments, review pertinent medical histories, and document findings accurately to inform treatment decisions.

  • Implement effective treatment protocols based on evaluation results while adjusting care strategies as required by the patient's progress.

  • Collaborate with multidisciplinary healthcare teams and make necessary referrals for further medical or rehabilitation consultations.

  • Document care activities consistently, from initial evaluations to discharge plans, ensuring all records reflect the patient's course of care.

  • Take ownership of daily patient treatments, maintaining high standards of care while maximizing departmental productivity.

  • Participate in team initiatives, contributing to the continuous improvement of access to physical therapy services.

  • Engage in professional development opportunities and attend relevant meetings and conferences to enhance your knowledge and skills.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, including those recovering from surgeries, managing chronic conditions, and dealing with post-injury rehabilitation. Our goal is to help each individual achieve optimum function and improve their overall quality of life through tailored physical therapy interventions.

Qualifications
  • Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.

  • Current and valid state license to practice physical therapy.

  • A minimum of two years of relevant experience in personal training or a related field involving patient care.

  • Valid CPR certification to ensure the safety of our patients.

  • Strong interpersonal skills with a proven ability to build rapport with patients and their families.

Preferred Experience
  • Experience working in home health settings or with digital health platforms is a plus, enhancing your ability to adapt treatments in diverse environments.

  • Active involvement in collaborative projects aimed at removing barriers to healthcare access is highly regarded.
