Key Responsibilities

• Conduct detailed Physical Therapy evaluations for children and adult patients, utilizing evidence-based assessment techniques.
• Develop individualized treatment plans grounded in clinical expertise and patient-specific goals, regularly monitoring their progress.
• Utilize the electronic health record system for documenting patient care, including comprehensive SOAP notes and progress summaries, ensuring compliance with deadlines.
• Engage in educational discussions with patients and families regarding treatment options, progress, and expectations throughout the healing process.
• Employ a proactive approach to re-evaluate treatment effectiveness, making necessary adjustments to treatment plans to ensure optimal patient outcomes.
• Achieve established utilization goals and contribute to business objectives, enhancing service delivery and organizational success.
• Collaborate effectively with multidisciplinary peers to coordinate care for clients utilizing multiple therapeutic services, promoting integrated health solutions.

Qualifications

Preferred Experience

• Experience in a rehabilitation setting is preferred.
• Familiarity with electronic health record systems and proficiency in digital documentation.

Compensation and Benefits
