Key Responsibilities

• Conduct comprehensive physical evaluations that assess range of motion, strength, balance, gait patterns, and functional limitations for each resident.
• Create and implement individualized treatment plans based on thorough assessments, establishing specific goals, exercise routines, and using modalities such as heat therapy or manual therapy.
• Administer therapeutic exercises to improve strength, flexibility, balance, and coordination, focusing on effective pain management strategies tailored to individual needs.
• Guide residents in walking techniques and the use of assistive devices like canes and walkers while addressing fall prevention strategies in every session.
• Support residents in activities of daily living (ADLs) such as dressing, bathing, and transferring, ensuring they can perform these tasks independently or with minimal assistance.
• Educate residents and their caregivers on the proper techniques for exercises, home exercise programs, and strategies for fall prevention.
• Maintain meticulous patient records, documenting progress notes, treatment plans, and functional assessments to track improvements and adjust treatment plans as necessary.

Patient Population / Client Focus

Our primary patient population consists of older adults living within an assisted living context. Many of our residents face age-related challenges such as arthritis, osteoporosis, dementia, and Parkinson's disease. Your expertise will directly contribute to enhancing their physical capabilities and overall well-being, helping them to engage in physical activity and maintain their independence.

Qualifications

• A valid Physical Therapy license in the state of New York.
• A Bachelor's Degree in Physical Therapy or a relevant field.
• Strong communication and interpersonal skills to build rapport with patients and their families.
• Ability to develop and implement patient-specific treatment plans that meet individual needs and goals.
• A genuine passion for helping patients achieve meaningful recovery and enhancing their quality of life.

Preferred Experience

• 2 years of clinical experience as a physical therapist, preferably in an assisted living or rehabilitation setting.
