Key Responsibilities

As a Physical Therapist at Confluent Rehabilitation, your responsibilities will include:

  • Developing and implementing tailored physical therapy treatment plans aimed at relieving pain, improving mobility, restoring muscle function, and preventing disability.
  • Overseeing the work of Physical Therapy Assistants, ensuring proper execution of treatment plans and adherence to prescribed therapy.
  • Conducting comprehensive evaluations of patients' physical abilities through diagnostic assessments to determine their individual needs and progress.
  • Assessing and recommending modifications to prosthetic and orthotic devices to enhance patient mobility and independence.
  • Delivering physical therapy services according to physician orders, including manual exercises, therapeutic modalities, and patient education on self-management techniques.
  • Observing and documenting patient progress, communicating effectively with interdisciplinary teams, and suggesting alterations in treatment plans based on patient responses.
  • Participating actively in case conferences to contribute to the overall treatment strategy and ensure seamless coordination of care.
  • Maintaining accurate clinical records, including initial evaluations, progress notes, and discharge summaries, adhering to compliance and regulatory standards.
Patient Population / Client Focus

You will work with patients recovering from various conditions including orthopedic injuries, post-surgery rehabilitation, neurological disorders, and chronic pain. Our focus is on ensuring each patient achieves the highest level of functional independence possible, with a keen emphasis on personalized therapy goals tailored to their unique challenges.

Qualifications

To succeed in this role, candidates must possess:

  • A Doctorate in Physical Therapy (DPT) from an accredited institution.
  • A valid PT license in the State of Florida, in good standing.
  • A minimum of two years of practical experience as a physical therapist, preferably in an acute care or home care setting.
  • Current CPR certification and a valid driver's license with automobile insurance.
  • Strong interpersonal and communication skills for effective patient and family engagement.
  • A commitment to clinical excellence and continuous personal and professional development.
Preferred Experience

While not mandatory, the following qualifications are preferred:

  • Experience in providing direct patient care in both clinical and home environments to enhance individualized care.
  • Familiarity with electronic medical record (EMR) systems to track patient progress and outcomes efficiently.
