Key Responsibilities
  • Conduct thorough evaluations to assess patients' physical abilities and develop personalized treatment plans based on findings, medical history, and therapy goals.
  • Engage and educate patients and their families about the evaluation results, treatment options, and expected outcomes, ensuring they are involved in setting realistic goals for recovery.
  • Implement various therapeutic interventions, leveraging your clinical judgement and expertise to adapt the treatment protocols as necessary based on patient progress and evolving healthcare standards.
  • Collaborate with other healthcare providers to discuss the patient's progress and modify care plans when required, ensuring comprehensive management of each patient's health.
  • Document all treatment-related activities meticulously and promptly, including patient evaluations, daily progress notes, and discharge summaries, utilizing our electronic health record (EHR) system effectively.
  • Maintain an organized and efficient workflow, meeting established productivity benchmarks while ensuring timely and professional communication with both patients and fellow team members.
  • Contribute to departmental goals through active participation in ongoing training and development programs, serving as a mentor to new staff and students as required.
  • Promote exceptional patient experiences through adaptive communication approaches, soliciting feedback, and addressing any barriers to access care.
Patient Population / Client Focus

Qualifications
  • A Bachelor's Degree in Physical Therapy from an accredited institution is required. A Doctorate in Physical Therapy (DPT) is preferred.
  • A valid state license to practice physical therapy in Connecticut is mandatory.
  • Certification in Basic Life Support (BLS) from the American Heart Association is required.
Preferred Experience
  • At least 2 years of hands-on physical therapy experience is preferred; however, recent graduates are also encouraged to apply as mentorship opportunities will be available.
  • Experience within a rehabilitation facility is highly desirable.
Compensation and Benefits
