Key ResponsibilitiesConduct thorough evaluations and assessments of patients' functional limitations, identifying key areas for intervention.
Develop individualized treatment plans that utilize evidence-based protocols, aligning with state practice acts and safety regulations.
Implement appropriate therapeutic interventions and techniques, adapting strategies as necessary based on patient progression and needs.
Document evaluations, treatment plans, and patient progress meticulously to maintain clarity and compliance within our evolving healthcare environment.
Collaborate with a multidisciplinary team of healthcare professionals to optimize patient outcomes through effective communication and joint decision-making.
Engage in patient education, empowering them with knowledge about their conditions and treatment goals, fostering adherence to prescribed regimens.
Lead discharge planning proactively, ensuring a smooth transition for patients into their homes or community-based care settings.
Participate in regular team meetings to discuss patient outcomes, share insights, and continuously improve processes that enhance access to therapy.
Your role will predominantly focus on outpatient orthopedic clients, ranging from post-operative rehabilitation to individuals with chronic pain or sports injuries. Our practice emphasizes personalized care, which allows you to make significant impacts on patients' functional independence and overall well-being. Your efforts will be crucial in guiding them through their rehabilitation journeys, supporting them every step of the way.
QualificationsDoctorate of Physical Therapy (DPT) from an accredited physical therapy program.
A valid state license to practice as a DPT in New Jersey.
Current CPR certification that ensures readiness for potential emergencies.
Minimum of two (2) years of experience in outpatient orthopedic rehabilitation, demonstrating sound clinical judgment and proficiency in treatment protocols.
Comfortable using electronic medical record (EMR) systems for documentation and patient management.
