Position Overview

Focus Physical Therapy is seeking a dedicated full-time Physical Therapist to enhance our rehabilitation efforts at our Rockford, Illinois facility. Work Environment

The successful candidate will join a vibrant team within our rehab center located in Rockford, Illinois. Our facility is designed to promote healing and recovery, equipped with state-of-the-art equipment and resources necessary for effective physical therapy. You will work onsite in a dynamic team environment, where patient care, continued learning, and teamwork are at the forefront of our operations. The schedule for this position is Monday through Friday, from 8:00 AM to 5:00 PM, with occasional flexibility to accommodate patient needs.

Key Responsibilities

• Implement individualized treatment plans that address the mobility, strength, and functional needs of each patient, considering their specific conditions and recovery goals.
• Guide patients through therapeutic exercises, stretches, and activities, educating them on the importance of these activities in promoting physical well-being and pain relief.
• Provide education to patients regarding body mechanics, correct exercise techniques, and home exercise programs to ensure progress outside of therapy sessions.
• Administer various modalities, including electrical stimulation, ultrasound, and heat/cold therapy, to enhance treatment efficacy.
• Monitor and document patients' progress meticulously, including tracking functional improvements, range of motion, and strength gains.
• Prepare treatment areas, ensuring that all equipment and supplies are organized and readily accessible to facilitate efficient therapy sessions.
• Work collaboratively with interdisciplinary teams, including Occupational Therapists and Speech-Language Pathologists, to ensure cohesive patient care and optimal clinical outcomes.
• Provide emotional support and positivity to patients as they navigate their rehabilitation journey, fostering an encouraging environment.
• Adhere to ethical and professional standards, maintaining patient confidentiality and respecting their rights throughout their treatment.

Patient Population / Client Focus

Our patients come from varied backgrounds, requiring rehabilitation for numerous conditions, including but not limited to post-operative recovery, sports injuries, age-related mobility issues, neurological disorders, and chronic pain management. As part of our committed team, you will play a significant role in empowering patients from diverse age groups, ensuring they receive personalized care geared towards meaningful recovery.

Qualifications

• A Bachelor's degree in Physical Therapy or a related field from an accredited institution.
• A valid state licensure or certification to practice as a Physical Therapist in Illinois.
• At least one year of hands-on experience in a rehabilitation setting is preferred, demonstrating effective patient management abilities.
• Strong communication skills and the ability to work collaboratively within a multidisciplinary team.
• Commitment to providing high-quality patient-centered care.

Preferred Experience

Experience working in a rehabilitation center, particularly with diverse patient populations across various rehabilitation needs, is highly desirable. Familiarity with electronic health records (EHR) and proven ability to document patient progress accurately will further enrich your application.

Compensation and Benefits
