Key Responsibilities

• Conduct comprehensive assessments of residents to determine their physical capabilities, specific rehabilitation needs, and overall treatment goals.
• Develop and customize individual treatment plans that enhance strength, mobility, and functional independence, utilizing evidence-based practices.
• Implement various rehabilitation modalities, including therapeutic exercises, manual therapy, and modalities such as ultrasound and electrical stimulation.
• Closely monitor resident progress, documenting outcomes through our electronic health record system, and adjusting therapy plans as necessary to ensure optimal care.
• Engage in regular collaboration with an interdisciplinary team, attending meetings and case conferences to discuss treatment updates and strategies.
• Educate residents and their families about their rehabilitation journeys, providing guidance on home exercise programs and measures to sustain physical health.
• Adhere to all regulatory compliance standards and maintain up-to-date knowledge of industry practices to ensure the highest quality of care is delivered.

Patient Population / Client Focus

As a Physical Therapist at Vital Motion, you will primarily work with elderly residents recovering from a variety of conditions, including post-surgical recovery, stroke rehabilitation, and chronic illness management. The role will require a keen understanding of the unique needs of geriatric patients, allowing you to adapt therapy approaches accordingly and foster a supportive, empathetic environment conducive to healing.

Qualifications

• A current California state licensure as a Physical Therapist.
• A Bachelor's degree in Physical Therapy from an accredited institution; a Master's or Doctorate is preferred.

Compensation and Benefits
