Key Responsibilities

As a Physical Therapist at Opti-Move Rehabilitation, your responsibilities will include:

• Conducting comprehensive evaluations of physical function and mobility for clients in their homes.
• Developing and implementing individualized treatment plans that address specific patient needs, ensuring achievable goals.
• Regularly reassessing patient progress through documented clinical observations to modify treatment plans as necessary.
• Completing all required documentation in a timely manner, which includes SOAP notes, progress summaries, and care plans using our electronic medical record system.
• Educating patients and their families on the recovery process and potential challenges, empowering them with knowledge and resources.
• Meeting minimum utilization goals and other business objectives aimed at enhancing our service delivery.
• Collaborating effectively with team members from various disciplines within Opti-Move to provide comprehensive care for clients who utilize multiple services.
• Championing the company's core values and embodying the organizational culture as an ambassador for Opti-Move Rehabilitation.

Patient Population / Client Focus

Qualifications
