Coordinates incoming calls from patients, family, physicians, and disciplines. Ensures that all personnel are informed of patient's hospitalization and follow-up with patients admitted to other hospitals. Coordinates small program(s) with limited budget/impact. Applies substantial knowledge and experience to perform a wide range of advanced activities and/or determines how to use resources to meet schedules and goals; serves as working supervisor for team or work group.<br> <br>Job Requirements:<br><br>Education and Work Experience:<br> <ul> <li>High School Education/GED or equivalent: Required</li> <li>Associate's/Technical Degree or equivalent combination of education/related experience: Preferred</li> <li>Three years' relevant healthcare experience: Preferred</li> </ul><br>Licenses/Certifications:<br> <ul> <li>Registered Nurse (RN) or Licensed Vocational Nurse (LVN) licensure in state of practice: Preferred</li> </ul><br>Essential Functions:<br> <ul> <li>Handles and coordinates incoming calls related to patients, physicians, referrals and agency services regarding physician orders, patient questions, and patient referrals.</li> <li>Facilitates telephone communications between, and demonstrates effective communication skills, with nursing, therapy, aide, social services, physicians, etc. regarding changes in patient/staff schedule, test results, etc.</li> <li>Assists with staffing/scheduling activities, soliciting input from managers.</li> <li>Provides follow-up for hospitalized patients, ensuring subsequent agency referral.</li> <li>Performs other job-related duties as assigned.</li> </ul><br>Organizational Requirements:<br> <br>Adventist Health is committed to the safety and wellbeing of our associates and patients.
