• Assist patients with daily living activities such as bathing, dressing, grooming, and eating, ensuring their comfort and dignity.


• Monitor and document patient health progress, reporting changes to the nursing staff and contributing to care plans.


• Provide emotional support to patients and families facing end-of-life decisions by listening attentively and offering companionship.


• Administer medications and treatments as directed by healthcare providers, ensuring compliance with care protocols.


• Coordinate closely with the hospice team, including social workers and nurses, to update care plans based on patient needs.


• Participate in team meetings to discuss patient progress, troubleshoot issues, and share insights to improve patient care.


• Utilize care management software to document care provided, track patient health data, and communicate effectively with the team.

The patients you will serve are individuals diagnosed with terminal illnesses, and they may face various physical, emotional, and psychological challenges during this crucial period of their lives. Working with patients and families who require sensitive and compassionate care is paramount. Your role will be key in ensuring enhanced quality of life, allowing patients to remain at home as they receive holistic support.

• A Bachelor’s Degree in Social Work and a valid nursing assistant certification are required.


• Strong communication and interpersonal skills to build effective relationships with patients and their families.


• Proficiency in using digital healthcare tools and software to document care and manage patient information.


• A compassionate nature with a commitment to providing quality care in line with hospice principles.


• Ability to work independently while also being part of a cohesive healthcare team.

• Previous experience as a CNA in hospice or palliative care is highly favorable.


• Experience working with patients from diverse backgrounds and understanding various cultural perspectives on end-of-life care.


• Familiarity with care management software and electronic health records.


• Training in grief counseling or emotional support techniques is a plus.
