Key Responsibilities

  • Conduct comprehensive assessments to identify each patient’s unique mental health needs and goals.

  • Engage in continuous communication with supervisors, colleagues, family members, and other stakeholders to ensure a coordinated approach to care.

  • Provide an organized schedule of patient appointments, home visits, and necessary observations to supervisors to facilitate effective monitoring of patient progress.

  • Coordinate detailed home visits designed to gather insights through in-home observations, chart reviews, and discussions with caregivers, ensuring a well-rounded understanding of patient situations.

  • Guarantee the effective implementation of individualized program plans through continued oversight and direct patient engagement, adjusting strategies as necessary to maximize outcomes.

  • Participate actively in Interdisciplinary Team Meetings and any relevant discussions that enhance collaborative care efforts.

  • Document detailed monthly and quarterly progress notes to track treatment progress and outcomes, maintaining clear communication with all involved parties.

  • Lead training sessions to ensure staff competency in program plan implementation and documentation processes, enhancing team capabilities and performance.

  • Provide ongoing support through follow-up interviews, home visits, and observations, dedicated to meeting the evolving needs of each individual.

  • Maintain organized records and documentation to foster transparency and facilitate easy access to vital information for interested parties.

  • Act as a liaison with the broader public and mental health community, reinforcing support for program development and improvement initiatives.

  • Assist in the creation and enhancement of training materials aimed at elevating the quality of care provided to individuals.

Patient Population / Client Focus

You will work with a diverse patient population, including both adults and youth, addressing a wide range of mental health challenges. Your expertise will particularly benefit individuals experiencing family dynamics, relationship stressors, trauma-related issues, and substance misuse challenges.

Qualifications

  • A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.

  • An active and unrestricted license to practice as a Professional Counselor in Georgia is essential.

  • A minimum of 2 years of professional counseling experience, particularly within a clinical setting, is necessary.

  • Specialization in areas such as family therapy, couples therapy, trauma recovery, or substance misuse is highly desirable.

  • Strong understanding of legal and ethical standards governing mental health practice.

  • Excellent interpersonal and communication skills, demonstrating the ability to establish rapport with a diverse clientele.

  • Proven collaboration skills, demonstrated through a history of successful teamwork in healthcare settings.

  • Proficiency with electronic medical record (EMR) systems and related technologies is required.
