Key Responsibilities
  • Conduct comprehensive assessments to thoroughly evaluate the mental health needs of each patient.
  • Facilitate effective communication among supervisors, direct care staff, families, and other stakeholders regarding patients' progress and treatment plans.
  • Prepare and provide a structured schedule to supervisors that includes appointments, home visits, and client assessments to monitor progress.
  • Coordinate and conduct home visits to gather observational data, along with conducting chart reviews and staff interviews.
  • Implement and ensure the effectiveness of treatment plans, making necessary adjustments based on patient feedback and progress.
  • Engage in interdisciplinary team meetings to discuss client care and share insights with team members.
  • Prepare and submit monthly and quarterly progress notes documenting clients' advancements.
  • Organize training programs that certify team members in the proper implementation of treatment plans and documentation practices.
  • Provide ongoing follow-up support through interviews and home visits to ensure the continued fulfillment of patients' needs.
  • Maintain organized records to ensure all client information is easily accessible to authorized parties.
  • Act as a liaison to the public and mental health community, fostering relationships to support development goals.
  • Contribute to the creation of training materials aimed at enhancing programs designed for client welfare.
Patient Population / Client Focus

In this role, you will work with a diverse population, including adults and youth, across various demographic and socioeconomic backgrounds. Our patients experience a range of challenges, including anxiety, trauma, and substance misuse, making your expertise pivotal in guiding them towards recovery and resilience.

Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
  • An active and unrestricted license as a Professional Clinical Counselor (LPCC) in the state of California.
  • A minimum of two years of counseling experience is necessary, ideally within a clinical setting.
  • Specialization in family therapy, couples therapy, trauma-informed care, or substance misuse is highly valued.
  • A strong understanding of legal and ethical standards in mental health practice.
  • Exceptional interpersonal and communication skills, enabling rapport-building with clients from various backgrounds.
  • Demonstrated collaborative ability while working within a multidisciplinary healthcare team.
  • Familiarity with electronic medical record (EMR) software and related technologies to enhance efficiency in documentation and client care.
Preferred Experience

While not mandatory, experience in specialized areas such as trauma-informed care or working with specific populations, such as youth or families, is advantageous. Familiarity with evidence-based treatment methodologies is also desirable to ensure quality service delivery.

Compensation and Benefits
