Key Responsibilities- Conduct comprehensive psychosocial assessments to evaluate behavioral health needs and formulate personalized treatment plans.
- Collaborate effectively with physicians and other healthcare professionals to address the psychosocial aspects of patient care.
- Utilize appropriate therapeutic techniques and interventions based on the best practices in psychosocial support and mental health treatment.
- Understand and apply cultural competencies to accommodate the diverse needs of children through to elderly patients in the hospice setting.
- Maintain thorough documentation of patient interactions, treatment plans, and evaluations in compliance with healthcare regulations and standards.
- Provide referrals to community resources to support patients and families as needed, ensuring a comprehensive approach to care.
You will work with a diverse demographic of patients, primarily focusing on those facing end-of-life challenges. This role requires a deep understanding of the emotional and psychological complexities that arise in patients and families navigating serious illnesses. Your expertise will help in providing therapeutic support and interventions aimed at enhancing quality of life during this critical time.
Qualifications- Master's Degree in Social Work (MSW) is required.
- Active state licensure as a Licensed Independent Clinical Social Worker (LICSW).
- At least 2 years of clinical experience in psychiatric social work, preferably in a healthcare or hospice setting.
- Current certification in safe management practices for handling disruptive and assaultive behavior.
- Exceptional communication skills, both written and verbal, to facilitate engaging discussions with patients and care teams.
- A strong ability to adapt to changing clinical situations and provide effective interventions under pressure.
- Experience working within hospice or palliative care settings.
- Specialized training in trauma-informed care or grief and bereavement support.
- Familiarity with electronic health records (EHR) systems and telehealth platforms.
- Annual salary range of USD 100,000 to 105,000, commensurate with experience.
