Key Responsibilities

• Conduct comprehensive assessments and develop tailored treatment plans, collaborating closely with clients to identify their unique needs and goals.
• Facilitate crisis intervention and risk assessment procedures, crafting pre-crisis and crisis plans that promote safety and well-being.
• Maintain timely and accurate documentation that adheres to regulatory standards, integrating evidence-based screening tools to guide your clinical practice.
• Collaborate with interdisciplinary teams to create and implement positive behavior support plans, ensuring holistic support for client progress.
• Serve as a liaison to community partners, state agencies, and service systems, fostering relationships that enhance client access to resources.
• Engage in continuous professional development through attendance at conferences and workshops, representing the clinic in various statewide initiatives.
• Promote the values and mission of South Coast Medical Clinic in every interaction, ensuring alignment with our commitment to community wellness and recovery.
• Complete other tasks as requested by the Clinic Director to support clinic operations and strategic initiatives.

Patient Population / Client Focus

Qualifications
