  • Graduate of an accredited Nurse Practitioner program with a Masters degree in Nursing REQUIRED
  • Nurse Practitioner Certification by ANCC or AANPCB REQUIRED
  • Most recent NPDB Self Query Report (within 30 days active) - REQUIRED
  • Unrestricted license to practice as a Physician Assistant in the State of Colorado REQUIRED
  • Must be eligible for DEA and Medicare/Medicaid provider status REQUIRED
  • Clean malpractice/license history - HIGHLY PREFERRED
  • Must have no sanctions/exclusions from HHS/OIG or GSA REQUIRED
  • Must hold BLS (note in "Additional Submission Details" field) - REQUIRED
  • Experience: 2 years of trauma surgery preferred OR completion of surgical fellowship - REQUIRED
  • Nurse Practitioner Certification by ANCC or AANPCB
  • Basic Life Support (BLS) AHA or American Red Cross
  • Unrestricted Physician Assistant license State of Colorado
