Certified Family Nurse Practitioner

Job Summary
We are seeking a dedicated and skilled Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will provide comprehensive healthcare services to patients of all ages, focusing on preventive care, health education, and management of acute and chronic conditions. The FNP will work collaboratively with physicians and other healthcare professionals to deliver high-quality patient care in a supportive environment.

Duties

• Conduct thorough health assessments and diagnostic evaluations for patients.
• Develop and implement individualized care plans based on patient needs.
• Administer medications as appropriate.
• Provide health coaching and education to patients regarding lifestyle changes and disease prevention.
• Assist in family planning services and provide guidance on reproductive health.
• Collaborate with multidisciplinary teams to coordinate patient care and improve outcomes.

Requirements

• Master’s degree in Nursing from an accredited program.
• Current state licensure as a Family Nurse Practitioner.
• Proven experience in a clinical setting, preferably in family practice or primary care.
• Strong skills in diagnostic evaluation and medication administration.
• Knowledge of utilization management practices is preferred.
• Experience with health coaching techniques is advantageous.
• Excellent communication skills, both verbal and written, with the ability to educate patients effectively.
