Work EnvironmentOur work setting is primarily remote, which allows flexibility while ensuring you are part of a close-knit, supportive team dedicated to exceptional patient care. Although the position is remote, you will collaborate closely with an interdisciplinary team of healthcare professionals, including physicians, nurses, and other therapists, to facilitate comprehensive care and promote positive patient outcomes.
Key Responsibilities- Conduct thorough assessments of patients' psychosocial needs, developing tailored treatment plans that address their unique challenges.
- Implement evidence-based therapeutic interventions, such as cognitive behavioral therapy and supportive counseling, to help patients cope with anxiety, depression, and trauma related to terminal illnesses.
- Maintain accurate and confidential documentation in compliance with state regulations and organizational policies, ensuring that patient records are up-to-date and secure.
- Collaborate effectively with the treatment team through regular meetings and case discussions to refine care plans and strategies that enhance patient support.
- Stay informed about the latest research in social work and hospice care to provide best-practice recommendations and improve service delivery.
In this role, you will primarily serve patients facing terminal illnesses, along with their families, providing counseling and guidance tailored to their emotional, social, and logistical needs during this challenging time. Your work will involve addressing complex issues ranging from grief and loss to caregiver support, ensuring that families receive the resources and assistance they require.
Qualifications- A Master’s degree in Social Work from an accredited institution is required.
- An active LCSW (Licensed Clinical Social Worker) license in the state of Georgia is mandatory.
- Demonstrable experience in clinical settings, specifically relating to hospice, palliative care, or gerontology is preferred.
- Strong communication, empathic listening, and interpersonal skills to effectively connect with patients and their families.
- Ability to work independently and as part of a collaborative team, demonstrating excellent problem-solving skills.
- Experience in providing care to patients with end-of-life issues, including knowledge of grief counseling techniques.
- Familiarity with electronic health record (EHR) systems and documentation practices.
- Previous involvement in community outreach programs or support groups for patients and their families.
