Key Responsibilities

  • Conduct comprehensive assessments to identify areas of concern in patients' mental health and develop individualized therapeutic treatment plans.

  • Collaborate with multidisciplinary teams to ensure the best outcomes for patient care, including healthcare professionals, hospice staff, and family members.

  • Provide treatment for a range of mental health issues including adjustment disorders, grief and loss, anxiety, depression, personality disorders, and trauma-related conditions such as PTSD.

  • Engage in regular check-ins with patients to monitor their mental state and adjust treatment plans as necessary.

  • Document patient interactions accurately and in a timely manner using electronic health record (EHR) systems to facilitate data tracking and analysis.

  • Provide crisis intervention and referrals to outside resources when appropriate to ensure holistic support for patients.

Patient Population / Client Focus

The primary focus of this role is to serve patients in hospice care, many of whom may experience a range of mental health challenges related to their illness, impending loss, and associated grief. You will work closely with patients to provide empathy, understanding, and effective coping strategies, reinforcing their dignity and enhancing their quality of life during critical periods. Your expertise will play a key role in promoting a supportive atmosphere that honors the complex emotional experiences of those at the end of life.

Qualifications

  • Must be a resident of California and licensed as a Licensed Clinical Social Worker (LCSW).

  • Possess a Master’s degree in Social Work, Counseling, Psychology, or a related mental health discipline.

  • Highly proficient in technology, particularly with electronic health record systems, ensuring smooth documentation and patient tracking.

  • Demonstrate excellent verbal and written communication skills, particularly in timely note-taking and documentation.

Preferred Experience

  • Prior experience in a hospice or palliative care setting, providing mental health support to terminally ill patients and their families.

  • Experience with cognitive behavioral therapy and trauma-informed care practices.

  • A track record of collaborating in a team-based approach to patient care, highlighting an ability to work harmoniously with diverse professionals.

Compensation and Benefits

We are proud to offer a competitive salary range of USD 80,000 to 122,000 per year for full-time positions, aligned with your qualifications and experience.
