This role will be primarily conducted in a telehealth environment, allowing counselors to connect with clients remotely while maintaining the flexibility to manage their schedules. • Conduct comprehensive assessments, allowing for tailored treatment plans based on individual patient needs.


• Serve as a key communicator, collaborating with supervisors, direct care staff, family members, and other stakeholders to update them on patient progress and treatment efficacy.


• Maintain an organized schedule for appointments, home visits, and follow-up observations to monitor client advancements.


• Coordinate home visits to facilitate necessary assessments, including chart reviews and staff interviews, ensuring thorough data analysis.


• Implement and ensure adherence to the developed treatment program plan, adjusting it based on patient feedback and progress.


• Engage in regular Interdisciplinary Team Meetings, contributing insights that inform patient care strategies and promote cohesive treatment approaches.


• Document monthly and quarterly progress notes, maintaining comprehensive records of patient advancements that can easily be accessed by interested parties.


• Facilitate training sessions to ensure staff members are equipped to implement program plans and documentation effectively.


• Provide ongoing follow-up support through interviews and virtual home visits, reinforcing treatment objectives and continuity of care.


• Actively participate as a liaison in the mental health community, promoting developmental goals and public outreach initiatives.

Your expertise will be focused on a diverse population, including both adults and youth facing various mental health challenges. You will play a crucial role in helping clients navigate issues such as anxiety, depression, relationship struggles, and trauma recovery. Specialization in family therapy, couples therapy, or substance misuse is highly valued, as it enhances our ability to cater to multifaceted client needs.

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.


• Possession of an active and unrestricted Professional Counselor (LPC) licensure in the state of California.


• Minimum of 2 years of relevant counseling experience, specifically in clinical contexts.


• Knowledge and adherence to legal and ethical standards of practice in mental healthcare.


• Exceptional interpersonal and communication skills, with an ability to build rapport with a diverse client base.


• Demonstrated capability to collaborate effectively within a multidisciplinary healthcare team.


• Proficient with electronic medical record (EMR) software, ensuring seamless integration of technology in patient care.

• Specialization in family therapy, couples therapy, trauma-informed care, or substance misuse treatment is preferred.
